Differences in the Effect of Using Sterile Water for Irrigation and Normal Saline Washing to Peritoneal Adhesion Post-Laparotomy on White Rats

BACKGROUND: Peritoneal adhesion is a side effect of abdominal surgery that often occurs. Many efforts were made in abdominal surgery to prevent or minimize the occurrence of this adhesion. One way to reduce the risk of bowel adhesions is through washing peritoneum method, so the differences in the use of washing solution on the incidence of adhesions after laparotomy surgery need to be investigated. The objective of this study is to determine differences in the effect of using sterile water and normal saline for washing irrigation to peritoneal adhesions after laparotomy in white rats.METHODS: This was an experimental study. Thirty-two sample of white rats were divided into two groups, group A and Group B, each group consists of 16 rats. Group A got washing using normal saline and group B got washing using sterile water for irrigation in laparotomy procedure to assess the peritoneal adhesion microscopically after relaparotomy. Peritoneal adhesion is one of the side effects that often occur after abdominal surgery.(1) Most of these conditions don't cause problems, but about of 5% of peritoneal adhesion cancause problems such as small bowel obstruction (SBO), infertility, pelvic and abdominal pain. These problems canRESULTS: This study was cunducted in April-May 2016. The founding of peritoneal adhesion after laparotomy in white rats using sterile water for irrigation in grade 1 were seven samples, grade 2 were five samples and grade 3 were four samples. Meanwhile, the founding of peritoneum adhesion after laparotomy using normal saline grade 1 are two samples, grade 2 are five samples, and grade 3 are nine samples. Statistically, significant differences were found (p<0.05).CONCLUSION: Some risk factors can lead to adhesion, such as trauma surgery, tissue ischemia, infection blood and foreign body irritating. Intraperitoneal irrigation with sterile water for irrigation is better than the use of normal saline in preventing peritoneal adhesion.

Intra Arterial Heparin Flushing Increases Cereberal Blood Flow in Chronic Ischemic Stroke Patients

BACKGROUND: Recently, stroke therapy is focused on reperfusion therapies for restoring cerebral blood flow (CBF) and minimizing the undesired effects of neuron ischemia. However, the thrombolytic therapy to restore CBF was restricted with narrow time window. On other hands, not many patients can reach the treatment immediately after the onset of stroke. A wider time window therapy that might increase CBF would probably helpful. This study aims to investigate the CBF improvement after intra arterial heparin flushing (IAHF) therapy in chronic stroke patients.METHODS: A clinical trial was conducted with time sampling. We collected chronic ischemic stroke subjects (with stroke onset ≥30 days) within periods February-September 2015. We investigated CBF before and after IAHF treatment in 75 chronic stroke patients. The difference before and after IAHF treatment in subgroup which is classified with infarct size and lesion area was tested. CBF was measured using MRI Quality Arterial Spin Labeling (qASL) with region of interest around infarct lesion.RESULTS: We found a significant CBF improvement (p<0.001) around infarct area after IAHF treatment with average 10.39mL/100g/min raised. CBF improvement was found in lacunar infarct (p<0.001) and non lacunar (p<0.001), also in infarct in cortical (p<0.05), subcortical (p<0.001) and both area (p<0.05).CONCLUSION: IAHF is associated with increased CBF around infarct area and IAHF probably offers some benefit for chronic stroke.

The Correlation Between Proprotein Convertase Subtilisin/Kexin Type 9 (PCSK9) and Insulin Resistance and the Components of Atherogenic Lipoprotein Phenotype in Males with Central Obesity

BACKGROUND: Proprotein Convertase Subtilisin/Kexin Type 9 (PCSK9) promotes the degradation of LDL receptor in hepatocytes. in vitro studies have proven that insulin increases the expression of PCSK9. Insulin resistance, a common condition in central obesity is characterized by dyslipidemia called Atherogenic Lipoprotein Phenotype (ALP). This study aimed to investigate the correlation between insulin resistance (HOMA-IR) and PCSK9, and to analyze whether this condition is related to the development of ALP in central obesity.METHODS: This is an observational study with crosssectional design. The subjects consisted of 62 male adults with central obesity, aged 30-60 years old. ELISA was used to measure plasma PSCK9.RESULTS: The mean plasma PCSK9 concentration of the samples was 283.7 ng/mL. PCSK9 had positive linear correlation with HOMA-IR (r=0.225, p=0.045) and Apo B (r=0.245, p=0.055). After controlling of HOMA-IR, PCSK9 had positive linear correlation with triglycerides (r=0.352, p=0.045). In population with HOMA-IR >2, crosstabs analysis showed that PCSK9 had significant correlation with triglycerides and Apo B with an odd ratio of 6,125 (r=0.376, p=0.037). Triglyceride showed significant negative correlation with HDL cholesterol and ratio of LDL/Apo B, but neither HOMA-IR nor PCSK9 did.CONCLUSION: In males with central obesity, PCSK9 is one of the factors mediating the occurence of ALP in insulin resistance.
